Community Care Co-ordinators play a central role in the success of Premier Community’s home care services. They are the people who bring together care delivery, people management, and planning, ensuring that our values are reflected in every visit we deliver.
Ensuring Safe, Person-Centred Care Everyday
Purpose of the role
Based primarily at our Head Office in Mansfield, this role sits at the heart of our Community Care operations, working closely with Registered Managers, Team Managers, and care teams to support both service users and staff.
The Community Care Co-Ordinator role exists to ensure that care is safe, consistent, person-centred, and well organised. Co-ordinators balance the needs of the people we support with the well-being of the staff who deliver care every day.
At Premier Community, care co-ordination is about more than scheduling visits. It’s about understanding individuals, responding to change, supporting teams, and making sure people can continue to live independently in their own homes.
Bringing People, Planning and Care Together
What a Community Care Co-Ordinator does
Community Care Co-Ordinators oversee the day-to-day co-ordination of care delivery across the community. Their responsibilities include:
Care Planning & Co-ordination
- Planning and managing weekly care rotas with a strong focus on logistics, efficiency, and continuity of care.
- Responding to changes and live issues to ensure visits are covered safely and promptly.
- Working closely with Registered Managers to support service user preferences and outcomes.
Supporting People & Teams
- Leading and supporting care staff in line with Premier Community policies and values.
- Ensuring staff feel listened to, supported, and part of a wider team.
- Inducting and supporting new team members as they settle into their roles and our culture.
- Supporting staff through probation reviews, performance discussions, and formal processes when required.
Communication & Collaboration
- Acting as a key point of communication between care teams, managers, and the wider organisation.
- Monitoring and responding to multiple communication channels in a professional and timely manner.
- Working collaboratively with other Team Managers to ensure a consistent and respectful approach across the business.
Quality & Presence in the Community
- Spending time in the community when required to observe practice and meet service users.
- Helping ensure care is delivered safely, respectfully, and in line with individual care plans.
