Our Hospital-to-Home Care service helps patients transition smoothly from hospital to home by focusing on several key areas. It starts with an assessment by the hospital’s Integrated Discharge Team, which creates a personalised care plan based on the your loved one’s s medical and personal needs. Our caregivers then step in to assist with daily tasks like bathing, dressing, and medication management, while working with GPs and nurses to meet any ongoing medical needs. For those requiring rehabilitation, therapy services such as physiotherapy can be arranged to support their recovery.

Emotional support is also provided, with our compassionate caregivers offering companionship to prevent loneliness during recovery. The service maintains regular communication with healthcare professionals to ensure any changes in the patient’s condition are quickly addressed, keeping their care on track.
