How to Access a Digital Care Plan
Step 1: Download the App
Head over to the relevant store for your device by clicking the links below:
Let Us Know Your Device ID
Step 2: Take a Screenshot of the App's Login Page
Once downloaded, please open the app directly from your device and take a screenshot of the initial login screen.
Your Device ID will be identified at the top of the screen (as per the example shown) and it is this information our team needs to register your device successfully.
Then fill in your details via the secure form below, and don’t forget to include the screenshot of your Device ID.
After your details have been submitted, our team will use these to process your request. Please note, we always complete a few checks before setting up a device to ensure you’re a known contact to the service user.

App Not Working?
If you have followed the steps above and received confirmation from our team that your device has been successfully registered, but you’re unable to log onto the app, please contact us directly via email to report an issue with your access.
Alternatively, if you have recently had a new device, we may need you to re-complete the above form to log your new details.
