When we talk about great care, it’s easy to focus on the big occasions. The special days out, major achievements and milestones that naturally make us want to celebrate. 

But some of the most meaningful examples of person-centred care happen in much quieter moments. 

A walk around the neighbourhood. Feeling fresh after a shower. Celebrating a birthday. Receiving some unexpected flowers. Enjoying something from the garden you love. 

Recently, five people supported by Premier Community reminded us why these everyday moments deserve to be celebrated too. 

Dave: Getting Out, Getting Active and Seeing Familiar Faces 

Dave has always enjoyed being active and was often seen walking around his local pit tip. 

As his mobility has changed, going out alone hasn’t always been appropriate. But that doesn’t mean something he enjoys should simply disappear from his life. 

With support from his carer, Dave has been enjoying shorter walks around lunchtime, giving him the chance to get out of the house, enjoy some fresh air and remain connected with his neighbourhood. 

An important part of those walks has been seeing familiar faces again. Dave enjoys stopping and talking to his neighbours, with one commenting on how lovely it was to see him out and about. 

It’s a great example of adapting support around an individual. The walk might look a little different today, but Dave can still enjoy being outdoors and part of the community he knows.

John: Celebrating an Everyday Achievement 

For John, a recent achievement happened much closer to home. 

John can sometimes feel unsure when moving around his flat, which has made having a shower something he doesn’t always feel confident attempting. 

With encouragement and the right support from his carer, Donna, John was able to enjoy a shower and was delighted with what he’d achieved. 

In John’s own words, he was happy to be “feeling fresh” after doing something he’d previously felt he couldn’t manage. 

It’s something many of us might do without a second thought. For John, it was an achievement worth recognising. 

Good home care should support people to do as much as they can for themselves, safely and with dignity. Sometimes that means knowing when to help, when to encourage and when to give someone the confidence to take the lead.

Barbara: 90 Years Worth Celebrating 🎂 

Some moments absolutely do call for a celebration. 

Barbara recently reached the wonderful milestone of her 90th birthday. 

Birthdays are part of the individual lives, histories and relationships of the people we support, and we’re always pleased when our care teams can share in those special occasions. 

So from all of us at Premier Community, a very happy 90th birthday to Barbara!

Brenda: Flowers, Just Because 💐 

There doesn’t always need to be an occasion to make somebody smile. 

Recently, Brenda’s care team surprised her with some flowers to brighten her day. 

It was a simple gesture, but one Brenda really appreciated. She was delighted with her flowers and couldn’t stop thanking the team for thinking of her. 

Relationships are an important part of home care. Taking time to know the person behind the care plan helps our carers notice the little things that might bring some extra happiness to someone’s day. 

Sometimes, a bunch of flowers is all it takes.

Jean: Bringing a Little of the Garden Closer 🌱 

Jean recently had an impressive birthday of her own, celebrating her 93rd. 

Nicola knew just how important Jean’s garden is to her. As Jean isn’t currently able to spend time in it in the way she once did, Nicola chose a particularly thoughtful birthday present: a plant. 

In a small but meaningful way, it brought a little piece of the garden Jean loves closer to her. 

That’s what knowing someone well can look like in practice. 

Person-centred care isn’t about assuming what will make somebody happy. It’s about listening, learning about their interests and recognising the things that are part of their identity.

Why the Small Moments Matter in Person-Centred Care 

These five stories are very different. 

For Dave, what mattered was getting outside and seeing his neighbours. For John, it was achieving something for himself. Barbara and Jean had milestone birthdays to enjoy, while an unexpected gesture brought a smile to Brenda’s day. 

None of their experiences need exaggerating to make them meaningful. 

Together, they demonstrate something important about high-quality home care: there is no single definition of a life well lived. 

Every person we support has their own routines, relationships, interests, ambitions and things that make a good day. 

Our role is to understand those things and provide care that protects dignity, encourages independence, supports choice and helps people remain connected with the lives they’ve built. 

Sometimes that creates a big Moment That Matters. 

And sometimes, the smallest moments matter just as much. 💙
